15% of $30 is $4.50, below the $5 floor, so the fee is $5. Total: $35, shipped free.
$80 at-cost medication
15% of $80 is $12, between the floor and cap, so that's the fee. Total: $92, shipped free.
$200 at-cost medication
15% of $200 is $30, above the $18 cap, so the fee is capped at $18. Total: $218, shipped free.

A fixed, capped fee removes any incentive tied to price or volume.
A percentage-based product markup would reward higher-cost prescriptions. A flat per-order membership would reward volume. Rivizy's fee is capped specifically so neither is true. The fee on a $200 prescription and a $600 prescription is the same $18.
